The purpose of this survey is to evaluate the effectiveness of your child's school in offering Career and Technical Education programs and communicating those options to parents. For security, you will be required to enter several fields pertaining to your child. Please have the following items available when submitting your survey results.

2. The last 4 digits of his/her Social Security Number
3. Your relationship with the child (parent or guardian, etc.)
