Career and Technical Education teachers are required to complete certain forms throughout the school year and submit them to the OCATE office.
Accessing the Templates
The templates for the various forms were created using Word 2000; therefore, you must be running a recent version of Word on your computer for these forms to work properly. You may download the blank form with a unique name to a local directory on your computer and complete it or you may complete the form online and then save it to your computer, but with a different filename.
Completing and Submitting the Forms
Each cell in each form has been formatted to accept the required data. Simply click in the shaded area within each cell and key the information. After the form is completely filled-out, you should send it to the OCATE office via an e-mail attachment.
